Application and Enrollment

To apply, send all of the following for your class in the same package to the registrar (we return incomplete submissions):

1	A printout of the application form, completely filled in: Wilderness First Aid Course Application Form for June 2009
2	Proof of prerequisite completion: CPR no longer ago than four years prior to the date of the WFAC (Send a copy, rather than the original, of your CPR card.)
3	Payment: Check payable to: WFAC 3-Day WFAC June 12-14, 2009 $205 Sierra Club member (include member number) $215 non member Click here for details on what the fee covers.
	Financial support for enrollment in wilderness first aid courses is available from the Sierra Club to Angeles Chapter leadership candidates. NOTE: The above-mentioned financial support is a Sierra Club (rather than a WFAC) function. Contact Tina of the Leadership Training Committee, rather than the WFAC, regarding this:
